Patients who participate in our Concierge Membership Plan experience:

  • No maximum per membership period
  • No deductible
  • No claims to submit
  • No pre-authorization
  • No limits for pre-existing conditions
  • Free consultations
  • No waiting period

Concierge Membership Plans

SINGLE WELLNESS MEMBERSHIP

Services Valued at $1100

For existing and new patients:

  • Comprehensive examination for new patients
  • Periodic Exam (2 per membership period)
  • 2 dental cleanings per membership period
  • Panorex or Full Series (1x per 5 years)
  • 1 set of Bitewing x-rays per membership period
  • Individual periapical x-rays
  • 1 emergency visit
  • 2 fluoride treatments per membership period
  • 15% off dental treatment
In-House Agreement

FAMILY WELLNESS MEMBERSHIP (2 MEMBERS)

For 2 Members ($420/yr Per Additional Member)

For existing and new patients:

  • Comprehensive examination for new patients
  • Periodic Exam (2 per membership period)
  • 2 dental cleanings per membership period
  • Panorex or Full Series (1x per 5 years)
  • 1 set of Bitewing x-rays per membership period

  • 3 Periodontal Cleanings included in this plan, 15% courtesy off additional cleanings
  • Panorex or Full Series (1x per 5 years)
  • 1 set of Bitewing x-rays per membership period
  • Individual Periapical x-rays
  • 1 emergency visit
  • 2 fluoride treatments per membership period
  • 1 site of Arestin per periodontal visit
  • 15% off dental treatment

CHILD MEMBERSHIP (AGES 3-15)

Services Valued at $900

For existing and new patients:

  • Comprehensive examination for new patients

Plan Information

Please note that there are no refunds for paid memberships, even if the plan benefits are not utilized during the participation period. The plan will be active for 12 consecutive months from the time the member joins.

Procedures that receive a 15% courtesy include, but are not limited to:

  • Crowns/ Bridges
  • Root Canal Therapy
  • Fillings and Core Buildups
  • Periodontics
  • Oral Surgery
  • Sealants
  • Dentures and Partials
  • Whitening Trays

Exclusions and Limitations:

Our Concierge Membership Plan is not an insurance plan, it has been designed to provide a way for our patients to obtain the dental care they need at the most affordable rate.

The membership plan:

  • Is standalone coverage not to be combined with insurance benefits.
  • If you choose to use Care Credit to pay for treatment, the office will pay the 10% interest for 12 months AND extend to you an additional 5% courtesy off your treatment for a total of 15%.
  • Cannot be used in a workman’s compensation treatment plan.
  • Cannot be combined with any other courtesies.
  • Is for use for treatments that lie within our realm of expertise.
  • Cannot be used for treatment with another dentist or specialist to whom we refer.
  • Cannot be used for dental products.
  • Benefits may only be utilized by the member and are not transferable.
